create a new image

create a new layer, and use the path-tool to create the first side of the box

now create 2 more layers and put on each layer one side of the box, shading it like you can see it here
4. Now fit the parts together, merge the layers down, if necessary clean up some disturbing pixels, drop a shadow and you are done
